Great Night Sky Psychotherapy PLLC

In accordance with Texas Health & Safety Code § 181.105, the following information is provided to explain how consumers may request their health care records, contact the applicable licensing authority, and file a consumer complaint.

Requesting Your Health Care Records

Current and former clients may request access to or copies of their health care records in accordance with applicable Texas and federal law.

To request your records from Great Night Sky Psychotherapy PLLC, please submit a written request by:

Secure Client Portal: Existing clients may submit a request through the secure client portal.

Please include enough information to identify the records you are requesting. Additional information or authorization may be required to verify your identity, process your request, or release records to another person or organization.

Requests for records are processed in accordance with applicable Texas and federal law. Any allowable fees will be communicated before records are released.
